HMO & Coverage What is an HMO and how do I check my benefits?
A Health Maintenance Organization (HMO) provides access to healthcare services under your membership plan. Check your benefit schedule or member handbook for covered services, benefit limits, exclusions, and any member share.
Ask your HR representative or HMO to confirm your current coverage before a planned visit.
HMO & Coverage How can I confirm that my HMO membership is active?
Check your HMO member portal or contact your HMO or HR representative. Have your member number and company name ready so they can verify your coverage dates and dependent enrollment.
A physical or digital card alone does not confirm that every service is covered.
HMO & Coverage How do I find an accredited doctor, clinic, or hospital?
Use your HMO provider directory, member app, or customer support channel. Confirm both the facility and the doctor are accredited for your specific plan. Check whether an appointment or Letter of Authorization (LOA) is required.
Verify accreditation before your visit because provider arrangements can change.
HMO & Coverage Are pre-existing conditions, medicines, and dental services covered?
Coverage depends on your plan. Pre-existing conditions may have separate limits or conditions. Medicines, dental care, maternity care, and preventive services may have their own benefits or exclusions.
Ask your HMO or HR representative to check the exact service against your benefit schedule.
HMO & Coverage Can I use my HMO benefits for my dependents?
A dependent must be enrolled and have active coverage under the applicable plan. Check the dependent member number, effective date, and benefits with your HR representative or HMO before requesting a service.
Dependent eligibility and enrollment periods depend on your company and provider arrangements.
HMO & Coverage What if my PhilHealth documents are incomplete at discharge?
Ask the hospital billing or PhilHealth desk which documents are missing and how they affect your bill before discharge. Also ask your HMO how your plan coordinates with PhilHealth benefits.
Do not assume your HMO will automatically cover a missing PhilHealth deduction. Any amount you need to settle depends on the hospital assessment and your plan.
LOA & Hospital Visits What is an LOA and how do I request one?
A Letter of Authorization (LOA) is an authorization for specified healthcare services, subject to your HMO coverage and approval. Prepare your member details, provider or hospital details, requested service, and any doctor request or supporting documents.
Open the NCIA LOA request form and complete the required fields. Keep your confirmation details for follow-up.
LOA & Hospital Visits What are common reasons for an LOA denial or delay?
- The service is excluded or exceeds a benefit limit.
- Membership is inactive or coverage has expired.
- The facility or doctor is not accredited for the plan.
- Member information is incomplete or does not match the records.
- A doctor request or supporting document is missing.
Ask for the specific reason and next steps. Correcting missing information does not guarantee approval for a service outside your coverage.
LOA & Hospital Visits Can I reuse an LOA or change the doctor, date, or procedure?
Check the approved services, provider, validity period, and conditions on your LOA. Contact the issuing HMO or helpdesk before changing the facility, doctor, procedure, or appointment date.
You may need an amended or new authorization. Do not assume an existing LOA covers a different service.
LOA & Hospital Visits What if I want to upgrade my room and board during confinement?
Ask hospital billing and your HMO for an estimate before choosing a room above your entitlement. Depending on your plan, you may need to pay the room difference and related charges, such as professional fees or procedure cost differences.
Confirm which charges you will shoulder before agreeing to the upgrade.
Claims & Reimbursement How do I submit a reimbursement claim?
- Confirm that the expense is eligible for reimbursement under your plan.
- Choose the correct provider and claim type in the reimbursement portal.
- Complete the required form and prepare supporting documents.
- Submit through the instructed channel and keep your acknowledgment or reference number.
Claims & Reimbursement What documents should I prepare for a claim?
Use the checklist for your provider and claim type. Common requirements include:
- A completed and signed reimbursement form.
- Itemized bills and official receipts or invoices required by the provider.
- A medical certificate, diagnosis, or clinical summary.
- Doctor requests, prescriptions, or test results when applicable.
- Member identification and payment details if requested.
Keep the originals. Your provider may require them even if you submit scans online.
Claims & Reimbursement What is the deadline for filing a reimbursement claim?
Filing deadlines vary by provider, plan, and claim type. Check the current claim form or provider instructions for the applicable deadline and how it is counted from treatment or discharge.
Prepare documents promptly. If a requirement is missing or you are close to the deadline, ask your provider for instructions rather than assuming an extension applies.
Claims & Reimbursement When will I receive my reimbursement or check?
Processing and payment timelines depend on your provider, claim type, completeness of documents, and evaluation outcome. Ask for the applicable turnaround time when your complete and valid requirements are acknowledged.
Keep your reference number for follow-up. Approval and payment release are separate steps, and the payment method depends on your provider.
Claims & Reimbursement How can I follow up on a claim or helpdesk ticket?
For an NCIA helpdesk ticket, choose Check Ticket Status in the navigation above and enter the Ticket ID and Reference Number from your confirmation email.
For the provider reimbursement decision or payment status, use the channel shown in your claim acknowledgment. Have your claim reference, submission date, and member details ready. A helpdesk ticket update does not by itself confirm claim approval.
Claims & Reimbursement What should I do if my claim is returned, denied, or only partly paid?
Review the provider explanation for missing documents, excluded services, benefit limits, or other reasons. Ask for clarification if the explanation is unclear, and submit corrections through the instructed channel.
To request a review, ask about the provider reconsideration process and any deadline. Keep copies of your documents and correspondence.
